I think is is geting tickled there cause she is ticklish on back of her legs and his hand is on the back of her knee tickling her there.
 
I'm 99% sure that picture (definitely real ZM) is related to when she injured her left knee during the shoot for her second video "Sometimes" back in February 1999.
 
I remember about the knee injury, but is it tickling? Or is she in a bit of pain from the check up. Sorry to be so doubtful, but I just wanna reassure.
 
It's nice to think it's a tickling pic nonetheless
