I apologize for not putting the form up on the first as I promised. My wife had some surgery done (nothing serious and she is fine, thanks for asking) so I have been kind of tied up doing the work of two people and taking care of three! LOL!

I will post the form and the explanaition today and I'll thank you now for all your input and help with the project!
