I thought David Starr and TerriTickle and David D’amato were all the same person. David D’amato supposedly died a few years ago.

No. Starr was a different producer. D'Amato was the criminal and used the pseudonym TerriTickle. And yes, D'Amato did actually die of, I believe, a heart attack shortly after HBO aired the documentary.
