form coding help needed

If any of our web wizards out there are familiar with creating online forms, could you please drop me a PM?

We're trying to get a form working that would send the completed information to our e-mail and then kick the person over to a confirmation page. We thought we had everything coded properly and in the right location. But, we can't get it working. Any help would be greatly appreciated!
