The lady in the chair is the Podiatrist. She gave her assistant instructions on how to apply some "light stimulation". The Podiatrist does some work on the patient as well.
 
Camry,that is a very good clip. Have you thought of changing the billing name from Tickle Heaven to something less obvious? I would love to download the clips you have.
