In interviews I have read with models, they actually enjoy using them. However it's all part of the package. A models fee can vary, but is usually all inclusive when it comes to the scene. Many years ago a friend and I made some very amateur bdsm videos. The models were paid and a fee was negotiated up front. It doesn't usually charge from then unless something else does.

This. I've occasionally had some models ask for more money if I were to do a certain thing to them (again, varies by model).

That said, one thing that's happened often enough for me to notice a trend is, a model will say something like, "This would be so much worse if I were blindfolded", and when I'd offer to do it they'd quickly backpedal and refuse. If memory serves, the only model who didn't back down was Jennifer.

That said, I'm shooting with Andi Vicious on the 2nd and we've done blindfold play before (off-camera) and I've purchased a few sleep masks since then, so odds are pretty good she won't refuse if I suggest it when the camera's rolling.

It's up to the model. From my knowledge they usually charge more for blindfolding or some types of gags, but they may not agree to be blindfolded with people they do not trust for example.

Generally, models will charge higher for things like nudity or for scenes they know will make them very uncomfortable or otherwise wouldn't do such as strict immobilization.

This is a reason why models do not generally publicize their rates in general. Sometimes it's case by case.
