Can anyone tell me who's foot is pictured in the circle in the upper left hand corner of the page just above the word "register"? Is that shot from a film?

It is from the FM concepts video "Your The Ticklish One" I beleive it's Holly Lane. You might what to check FM's site.
 
Longbaugh is correct. It is Holly Lane.
