Model Release / Consent Form

This has probably been asked before, but do any producers have a model release and or consent form that you'd be willing to share? It would be much appreciated.

Sure. I've been using this one for nigh on a decade. Pornhub offers one in their vendors section as well that's a little more comprehensive. You'll also need a 2257 form. I have one of those too, if you want me to follow up.
