This is coming from a person who knows how to safely do sounding and has sounded many times. I also use metal sounds made out of surgical grade metal. Using a fingernail doesn't seem like a good idea. First off you would probably want to use gloves and if you didn't use gloves you better hope the person's hands and more importantly nails are super clean.
