I think it's safe to say that the overwhelming majority of producers use model releases. Aside from very special cases it would be very stupid and dangerous not to use them.
The release I use is pretty standard. It could be used for most types of video shoots and could easily be modified for photo shoots. It doesn't include any specifics about tickling, bondage or other fetish stuff. It simply isn't necessary (though some producers may feel otheriwse). It has a number of clauses, like the model declares that she's 18+, that she authorizes me to capture the shoot on film, that I have the ownership rights over the footage, that I can sell/distribute it (and use material extracted from it for promotional purposes), that she receives a certain amount of money for her participation, that I can't modify the footage in a major way (a silly clause, really, and I always mentally shake my head and roll my eyes when someone asks me if I can paste their head on a naked body or something like that), that she frees me of responsibility if something goes wrong, that sort of thing. I get the feeling that you would find most model releases to be pretty generic and boring.
